Carson Pirie Scott Furniture

5 years ago, I bought a chair from Carson Pirie Scott - I chose a fabric and had custom made. Now, I would buy the ottoman, but I do not live in an area with Carson. I wish I could find the make / model of the chair and order from the manufacturer or take in another furniture store / and see if they could order it. But the tags have nothing recognizable as a brand / model, just a few codes / numbers.

I thought I'd give it a shot and see if anyone here knew anything about it until I called Carsons or just put the tags in a furniture store. Ideas? Are there any websites that might be able to help me (for example, places of custom furniture)?
